PRE-DISCHARGE
We’ll discuss your transition with your hospital’s primary care physician and make sure you’re ready to return home, while your team helps monitor your health during this process.
POST-DISCHARGE
After your return, a New Riverdale social worker will conduct a routine check-up during the first few years post-discharge
